Narrative:

I was providing radar OJT on sbv lo. At the time of the occurrence, there were approximately 5 aircraft in the sector. The radar developmental removed the flight plan on an aircraft who had just cancelled IFR following an ILS approach to dan. Seconds before this, Y called on to the frequency requesting advisories. I observed the developmental remove the flight plan on aircraft as I moved to the d-side position to get a flight plan readout on Y. As I did this, the developmental removed the flight plan at xy:min:seconds on small transport X. I did not see this entry. When Y called back a second time requesting advisories, it sounded like his call sign was Z therefore, I did a flight plan readout at d-zz on wrong call sign. Next, the developmental, using the r-qak beacon key, assigned a beacon code to wrong call sign. I was still sitting next to the developmental at the d-zz position. When the developmental assigned code to wrong call sign. We learned the call sign was in fact Y. I then entered messages from the d-side position to assign a code to Y and began entering a flight plan on Y. Again, traffic was light, and since we had no d-side, I felt there was no problem with my OJT-ing and entering a flight plan. While entering the flight plan on Y, I received a message stating '1b1' not stored. I asked Y the name of the VOR closest to his destination. This distraction my attention away from the radar for a short period. I entered a new flight plan using Y and received an accept fp message on Y. At about this time, the developmental issued an approach clearance to military aircraft, using improper phraseology. I instructed him as to the proper approach items to be issued and the phraseology to be used. By this time the limited data block on small transport X was in gso approach airspace without my knowledge. I failed to observe the deviation due to the following: the developmental removed the fp on small transport X for no reason, without my knowledge. The distraction of entering a fp on Y kept me from thoroughly scanning the sector for the period of time small transport Y was untracked. My attention to the approach clearance issued to military aircraft and the subsequent instruction that followed prevented me from scanning the sector at the time Y was untracked.

I DID NOT SEE THIS ENTRY. WHEN Y CALLED BACK A SECOND TIME REQUESTING ADVISORIES, IT SOUNDED LIKE HIS CALL SIGN WAS Z THEREFORE, I DID A FLT PLAN READOUT AT D-ZZ ON WRONG CALL SIGN. NEXT, THE DEVELOPMENTAL, USING THE R-QAK BEACON KEY, ASSIGNED A BEACON CODE TO WRONG CALL SIGN. I WAS STILL SITTING NEXT TO THE DEVELOPMENTAL AT THE D-ZZ POS. AGAIN, TFC WAS LIGHT, AND SINCE WE HAD NO D-SIDE, I FELT THERE WAS NO PROBLEM WITH MY OJT-ING AND ENTERING A FLT PLAN. WHILE ENTERING THE FLT PLAN ON Y, I RECEIVED A MESSAGE STATING '1B1' NOT STORED. I ASKED Y THE NAME OF THE VOR CLOSEST TO HIS DEST. THIS DISTR MY ATTN AWAY FROM THE RADAR FOR A SHORT PERIOD. I ENTERED A NEW FLT PLAN USING Y AND RECEIVED AN ACCEPT FP MESSAGE ON Y. AT ABOUT THIS TIME, THE DEVELOPMENTAL ISSUED AN APCH CLRNC TO MIL ACFT, USING IMPROPER PHRASEOLOGY. I INSTRUCTED HIM AS TO THE PROPER APCH ITEMS TO BE ISSUED AND THE PHRASEOLOGY TO BE USED. BY THIS TIME THE LIMITED DATA BLOCK ON SMT X WAS IN GSO APCH AIRSPACE WITHOUT MY KNOWLEDGE. I FAILED TO OBSERVE THE DEV DUE TO THE FOLLOWING: THE DEVELOPMENTAL REMOVED THE FP ON SMT X FOR NO REASON, WITHOUT MY KNOWLEDGE. THE DISTR OF ENTERING A FP ON Y KEPT ME FROM THOROUGHLY SCANNING THE SECTOR FOR THE PERIOD OF TIME SMT Y WAS UNTRACKED. MY ATTN TO THE APCH CLRNC ISSUED TO MIL ACFT AND THE SUBSEQUENT INSTRUCTION THAT FOLLOWED PREVENTED ME FROM SCANNING THE SECTOR AT THE TIME Y WAS UNTRACKED.
