Narrative:
Jan/xx/92 I believe there was a loss of separation between air carrier X and air carrier Y. Departed teb and air carrier Y departed isp. The loss occurred on the boundary between liberty east working air carrier X and jfk departure working air carrier Y. I feel the loss of separation was due to a late transfer of control (frequency change). I did not talk to air carrier Y until he was within 1/2 mi from my airspace.

Title: ACR X HAD LTSS FROM ACR Y. SYS ERROR.
Narrative: JAN/XX/92 I BELIEVE THERE WAS A LOSS OF SEPARATION BTWN ACR X AND ACR Y. DEPARTED TEB AND ACR Y DEPARTED ISP. THE LOSS OCCURRED ON THE BOUNDARY BTWN LIBERTY E WORKING ACR X AND JFK DEP WORKING ACR Y. I FEEL THE LOSS OF SEPARATION WAS DUE TO A LATE TRANSFER OF CTL (FREQ CHANGE). I DID NOT TALK TO ACR Y UNTIL HE WAS WITHIN 1/2 MI FROM MY AIRSPACE.
