Narrative:

Scheduled flight from dtw-mia. Proceeding to rsw then CCE2 arrival to mia. Mia airport closed due to thunderstorm over and around the field. Approximately 100 mi north of rsw told to expect holding. Anticipated holding for 1 hour so I left flight deck for mens room. I resumed flying the aircraft and was cleared to hold at collier county (cce VOR) as published. The first officer and I discussed the fact that the holding radial on the chart was not clearly specified. I checked the low and high altitude charts without satisfaction so we checked with mia center to confirm cleared to hold northwest on the 288 degree radial. ATC verified this. The so and I had been discussing our fuel state and conferring with flight dispatch over our possible holding time and we changed our alternate from mco to fll to allow a longer hold. The first officer and I discussed holding pattern entry and direction of hold and I entered hold over lee county (rsw VOR). First officer reported entering hold at FL290. Mia center responded 'understand entering hold at collier county.' first officer replied 'affirmative.' first officer and I discuss why would center respond in this manner and we decide it's because he called entering hold about 1.5-2 mi prior to actual fix. My radios were set up for rsw. First officer radios set up cce. I had just turned inbound from my 20 mi outbound leg when first officer advises I'm at wrong fix. I proceed to correct pattern at cce (approximately 23 mi due south) and nothing had been said from center. Our error was at XX00Z, at XX10Z air carrier X airlines started to commit exact error but corrected themselves. Performance considerations: I did not fully communicate with my first officer upon return from biffy. I got involved with so and dispatch at a very busy and inopportune time. I don't think the holding pattern at cce is well defined. Lee county and collier county sound very similar at 500 KTS and only 23 mi apart. Mia center didn't keep a very close eye on us or air carrier X.

Title: ACR LGT ENTERED AHD AT THE WRONG VOR ENRTE TO MIA.

Narrative: SCHEDULED FLT FROM DTW-MIA. PROCEEDING TO RSW THEN CCE2 ARR TO MIA. MIA ARPT CLOSED DUE TO TSTM OVER AND AROUND THE FIELD. APPROX 100 MI N OF RSW TOLD TO EXPECT HOLDING. ANTICIPATED HOLDING FOR 1 HR SO I LEFT FLT DECK FOR MENS ROOM. I RESUMED FLYING THE ACFT AND WAS CLRED TO HOLD AT COLLIER COUNTY (CCE VOR) AS PUBLISHED. THE FO AND I DISCUSSED THE FACT THAT THE HOLDING RADIAL ON THE CHART WAS NOT CLRLY SPECIFIED. I CHKED THE LOW AND HIGH ALT CHARTS WITHOUT SATISFACTION SO WE CHKED WITH MIA CENTER TO CONFIRM CLRED TO HOLD NW ON THE 288 DEG RADIAL. ATC VERIFIED THIS. THE SO AND I HAD BEEN DISCUSSING OUR FUEL STATE AND CONFERRING WITH FLT DISPATCH OVER OUR POSSIBLE HOLDING TIME AND WE CHANGED OUR ALTERNATE FROM MCO TO FLL TO ALLOW A LONGER HOLD. THE FO AND I DISCUSSED HOLDING PATTERN ENTRY AND DIRECTION OF HOLD AND I ENTERED HOLD OVER LEE COUNTY (RSW VOR). FO RPTED ENTERING HOLD AT FL290. MIA CENTER RESPONDED 'UNDERSTAND ENTERING HOLD AT COLLIER COUNTY.' FO REPLIED 'AFFIRMATIVE.' FO AND I DISCUSS WHY WOULD CENTER RESPOND IN THIS MANNER AND WE DECIDE IT'S BECAUSE HE CALLED ENTERING HOLD ABOUT 1.5-2 MI PRIOR TO ACTUAL FIX. MY RADIOS WERE SET UP FOR RSW. FO RADIOS SET UP CCE. I HAD JUST TURNED INBOUND FROM MY 20 MI OUTBOUND LEG WHEN FO ADVISES I'M AT WRONG FIX. I PROCEED TO CORRECT PATTERN AT CCE (APPROX 23 MI DUE S) AND NOTHING HAD BEEN SAID FROM CENTER. OUR ERROR WAS AT XX00Z, AT XX10Z ACR X AIRLINES STARTED TO COMMIT EXACT ERROR BUT CORRECTED THEMSELVES. PERFORMANCE CONSIDERATIONS: I DID NOT FULLY COMMUNICATE WITH MY FO UPON RETURN FROM BIFFY. MIA CENTER DIDN'T KEEP A VERY CLOSE EYE ON US OR ACR X.
