Narrative:

I was training on south radar. I had air carrier X just south of the localizer that was northbound that I cleared for the approach. He didn't take the turn so I cleared the aircraft again. He still didn't turn. The north controller had air carrier Y he was vectoring for the approach, when the north controller saw my aircraft going through the localizer he turned air carrier Y northeast. I then turned air carrier X southeast. The problem was equipment. The button that activates the landline between us and kalamazoo was punched in, but the light that indicates so was burned out. When this line is punched in no transmissions go out to aircraft. Supplemental information from acn 187798. Air carrier Y on approach to grr. Controller said turn left to heading and climb to 4000 ft, traffic at our 1 O'clock position. TCAS also showed traffic. Approach supervisor said air carrier X did not acknowledge call for turn to final.

Title: ACR X HAD LTSS FROM ACR Y. SYS ERROR.

Narrative: I WAS TRAINING ON S RADAR. I HAD ACR X JUST S OF THE LOC THAT WAS NBOUND THAT I CLRED FOR THE APCH. HE DIDN'T TAKE THE TURN SO I CLRED THE ACFT AGAIN. HE STILL DIDN'T TURN. THE N CTLR HAD ACR Y HE WAS VECTORING FOR THE APCH, WHEN THE N CTLR SAW MY ACFT GOING THROUGH THE LOC HE TURNED ACR Y NE. I THEN TURNED ACR X SE. THE PROBLEM WAS EQUIP. THE BUTTON THAT ACTIVATES THE LANDLINE BTWN US AND KALAMAZOO WAS PUNCHED IN, BUT THE LIGHT THAT INDICATES SO WAS BURNED OUT. WHEN THIS LINE IS PUNCHED IN NO TRANSMISSIONS GO OUT TO ACFT. SUPPLEMENTAL INFO FROM ACN 187798. ACR Y ON APCH TO GRR. CTLR SAID TURN L TO HDG AND CLB TO 4000 FT, TFC AT OUR 1 O'CLOCK POS. TCAS ALSO SHOWED TFC. APCH SUPVR SAID ACR X DID NOT ACKNOWLEDGE CALL FOR TURN TO FINAL.
