Narrative:

While working departure control, I radar idented air carrier X, reclred air carrier X from an assigned altitude of 5000 ft to 10000 ft, and vectored aircraft to join a STAR. Air carrier X read back all instructions, including an altitude of 11000 ft. I missed the incorrect readback, and switched air carrier X to the center as he climbed out of 8000 ft. Supplemental information from acn 180338: air carrier X departing runway 6 with a clearance of runway heading and 5000 from rsw. About 2 mins later, tower told us to contact rsw departure. I contacted rsw departure reporting to them that we were passing 1800 for 5000 runway heading. His response was radar contact, turn right to a heading of 140, climb and maintain 10000 ft. However, I thought he said climb and maintain 11000 ft. My response to rsw departure was to turn right to a heading of 140 and continue the climb to 11000 ft. Rsw departure controller didn't respond to my reply of 11000. At about 8000 ft rsw departure told us to contact mia center. I contacted mia center reporting that we were passing 9000 for 11000. Mia center didn't respond so we continued the climb to 11000 ft. At 10900 mia center asked us what altitude we were at. I responded that we were at 10900 and level off at 11000. Mia center said that we were only cleared to 10000 ft by rsw departure and to descend to 10000 ft. We descended to 10000 ft and were then handed off to mia approach who then told us to call mia center when we got on the ground. I called mia center after we landed at mia airport. The supervisor stated that there was an altitude deviation and asked if I could call back because they were listening to the tapes. When I called back 2 hours later, the mia center supervisor said that I did reply 11000 however we were only cleared to 10000 but because rsw departure and mia center never replied to my response of 11000 we were now cleared to 11000. About 2 hours later another supervisor called to tell me that I may get a letter from rsw or mia asking for my side of the story. I was also told that the mia controller was working multiple sectors, he was on speaker not earphone, and that the controller's assistant was away because they had 2 targets from havana airspace entering united states airspace with no identify and was trying to find out who or what the 2 targets were. Supervisor also stated that neither controller (rsw nor mia) responded to my reply of 11000.

I CONTACTED RSW DEP RPTING TO THEM THAT WE WERE PASSING 1800 FOR 5000 RWY HDG. HIS RESPONSE WAS RADAR CONTACT, TURN R TO A HDG OF 140, CLB AND MAINTAIN 10000 FT. HOWEVER, I THOUGHT HE SAID CLB AND MAINTAIN 11000 FT. MY RESPONSE TO RSW DEP WAS TO TURN R TO A HDG OF 140 AND CONTINUE THE CLB TO 11000 FT. RSW DEP CTLR DIDN'T RESPOND TO MY REPLY OF 11000. AT ABOUT 8000 FT RSW DEP TOLD US TO CONTACT MIA CENTER. I CONTACTED MIA CENTER RPTING THAT WE WERE PASSING 9000 FOR 11000. MIA CENTER DIDN'T RESPOND SO WE CONTINUED THE CLB TO 11000 FT. AT 10900 MIA CENTER ASKED US WHAT ALT WE WERE AT. I RESPONDED THAT WE WERE AT 10900 AND LEVEL OFF AT 11000. MIA CENTER SAID THAT WE WERE ONLY CLRED TO 10000 FT BY RSW DEP AND TO DSND TO 10000 FT. WE DSNDED TO 10000 FT AND WERE THEN HANDED OFF TO MIA APCH WHO THEN TOLD US TO CALL MIA CENTER WHEN WE GOT ON THE GND. I CALLED MIA CENTER AFTER WE LANDED AT MIA ARPT. THE SUPVR STATED THAT THERE WAS AN ALT DEV AND ASKED IF I COULD CALL BACK BECAUSE THEY WERE LISTENING TO THE TAPES. WHEN I CALLED BACK 2 HRS LATER, THE MIA CENTER SUPVR SAID THAT I DID REPLY 11000 HOWEVER WE WERE ONLY CLRED TO 10000 BUT BECAUSE RSW DEP AND MIA CENTER NEVER REPLIED TO MY RESPONSE OF 11000 WE WERE NOW CLRED TO 11000. ABOUT 2 HRS LATER ANOTHER SUPVR CALLED TO TELL ME THAT I MAY GET A LETTER FROM RSW OR MIA ASKING FOR MY SIDE OF THE STORY. I WAS ALSO TOLD THAT THE MIA CTLR WAS WORKING MULTIPLE SECTORS, HE WAS ON SPEAKER NOT EARPHONE, AND THAT THE CTLR'S ASSISTANT WAS AWAY BECAUSE THEY HAD 2 TARGETS FROM HAVANA AIRSPACE ENTERING UNITED STATES AIRSPACE WITH NO IDENT AND WAS TRYING TO FIND OUT WHO OR WHAT THE 2 TARGETS WERE. SUPVR ALSO STATED THAT NEITHER CTLR (RSW NOR MIA) RESPONDED TO MY REPLY OF 11000.
