Narrative:

Military helicopter (mlt) departed fbg at xx:34Z, IFR to dov via V54 iso at 5000'. He contacted fay approach at 2000' negative transponder. After several times of trying to recycle, he was told transponder appears malfunctioning. Mlt reported reaching 5000' as instructed. After 20 mins of working traffic, I noticed flight progress strip on mlt west/O proper notation of a handoff accomplished with gsp approach. The aircraft was still on frequency and reported iso VOR. Late coordination was accomplished with no loss of sep.

Title: DEP CTLR FORGOT ABOUT A NON TRANSPONDER MLT ON AIRWAY. ACFT ENTERED ADJACENT TRACON AIRSPACE WITHOUT RADAR HANDOFF.

AFTER 20 MINS OF WORKING TFC, I NOTICED FLT PROGRESS STRIP ON MLT W/O PROPER NOTATION OF A HDOF ACCOMPLISHED WITH GSP APCH. THE ACFT WAS STILL ON FREQ AND RPTED ISO VOR. LATE COORD WAS ACCOMPLISHED WITH NO LOSS OF SEP.
