Narrative:

I was at 2500' near ind when I decided to pick up a clearance. Level at 3000' when I lost 500' while IFR and center notified me. At 2500' I saw the ground and cancelled the IFR flight plan and returned to ind. Landed and picked up 20 gals of fuel. Small traces of ice removed. Checked WX and departed IFR to sdf. I believed there was an inversion and no ice would be encountered. At 500' (an inversion) was the altitude I thought best with warm air aloft. Ice was picked up and I returned to ind and lined up for 5R while cleared for 5L. I finally realized I was lined up for 5R and changed to 5R. On departure from 5 the controller told me to turn right to 140 degrees and I turned left to 140 degrees. I had no current maps or approach plates because I accidentally picked up my old maps from home and didn't pick up my new maps and approach plates from the airport hangar on my first departure from dpa. There were current maps and approach plate sitting on the table in the hangar at dpa. I failed to set the dg properly and was off 15 degrees on my heading. The dg was checked on the ground and worked fine. Also for the ice a PIREP said no ice encountered at 5000', an when I departed a second time I believed no ice would be encountered.

Title: ALT DEVIATION, WRONG CHARTS, ICE ENCOUNTER, WRONG RWY APCH, WRONG DIRECTION TURN AFTER TKOF, DG MISSET AND FLEW 15 DEGREES OFF HEADING. AN ALL AROUND BAD DAY.

I BELIEVED THERE WAS AN INVERSION AND NO ICE WOULD BE ENCOUNTERED. AT 500' (AN INVERSION) WAS THE ALT I THOUGHT BEST WITH WARM AIR ALOFT. ICE WAS PICKED UP AND I RETURNED TO IND AND LINED UP FOR 5R WHILE CLRED FOR 5L. I FINALLY REALIZED I WAS LINED UP FOR 5R AND CHANGED TO 5R. ON DEP FROM 5 THE CTLR TOLD ME TO TURN RIGHT TO 140 DEGS AND I TURNED LEFT TO 140 DEGS. I HAD NO CURRENT MAPS OR APCH PLATES BECAUSE I ACCIDENTALLY PICKED UP MY OLD MAPS FROM HOME AND DIDN'T PICK UP MY NEW MAPS AND APCH PLATES FROM THE ARPT HANGAR ON MY FIRST DEP FROM DPA. THERE WERE CURRENT MAPS AND APCH PLATE SITTING ON THE TABLE IN THE HANGAR AT DPA. I FAILED TO SET THE DG PROPERLY AND WAS OFF 15 DEGS ON MY HDG. THE DG WAS CHKED ON THE GND AND WORKED FINE. ALSO FOR THE ICE A PIREP SAID NO ICE ENCOUNTERED AT 5000', AN WHEN I DEPARTED A SECOND TIME I BELIEVED NO ICE WOULD BE ENCOUNTERED.
