Narrative:

Altitude restriction: approaching ogden VOR, we were given a crossing restricion of ogden 25 NM/17000'/250 KTS. Ogden DME was lost during descent and restriction was not made. ATC was notified of DME loss and that we were unable to meet the restriction. ILS approach salt lake city. ATIS C was received stating ILS 31L was in use at slc. ILS 34L approach was briefed and cockpit was set up. During this time DME was lost and we were concerned about crossing restriction. After passing ogden, we were given to approach and given a vector to a point not on the approach for 34L. We questioned the fix and were told the runway in use was 16R.

Narrative: ALT RESTRICTION: APCHING OGDEN VOR, WE WERE GIVEN A XING RESTRICION OF OGDEN 25 NM/17000'/250 KTS. OGDEN DME WAS LOST DURING DSNT AND RESTRICTION WAS NOT MADE. ILS 34L APCH WAS BRIEFED AND COCKPIT WAS SET UP. DURING THIS TIME DME WAS LOST AND WE WERE CONCERNED ABOUT XING RESTRICTION. AFTER PASSING OGDEN, WE WERE GIVEN TO APCH AND GIVEN A VECTOR TO A POINT NOT ON THE APCH FOR 34L. WE QUESTIONED THE FIX AND WERE TOLD THE RWY IN USE WAS 16R.
