Narrative:

The captain was hand-flying the advanced technology widebody transport. Autoplt was not engaged. As we passed FL300 for FL310 I called '1000' to go.' captain acknowledged. ZMA then cleared us 'present position direct aex.' I started to put the new route into the route 2 so original route (#1) could be retained if clearance changed back. The captain was watching what I was doing. He asked why I was using route 2. The altitude alert warning sounded before I could answer. I called 'level off.' we were at FL315. ZMA advised us to maintain FL310, traffic 1 O'clock, 20 mi. The captain did not follow company procedures and his own briefing: 'the PF will fly the aircraft; the PNF will handle the radios, CDU, navigation,' etc.
