Narrative:

On takeoff from bzn experienced engine failure and fire on #3. Started a fuel dump, leveled aircraft at 800' AGL to retract flaps. Advised bzn FSS we had an engine failure and fire and would be returning to land bzn runway 12. Also asked to have the equipment standing by. Started to run checklists. On emergency checklist followed instructions and fired 2 bottles into #3 engine. Fire light did not go out and fire bell kept ringing intermittently. Terminated dump and started to turn final just outside OM at bzn. Completed all checklists and asked FSS if he could see either smoke or fire coming out of #3 engine. Was advised by either FSS or taxiing aircraft #3 engine was still on fire. Told FSS to have equipment meet the aircraft after landing and that I planned to evacuate the aircraft on runway. Advised F/a's nd passengers on P/a to plan evacuation after we stopped aircraft. On the runway since by that time we were on short final. Landed aircraft and after it was stopped I called for evacuation on the left side only. The fire department immediately reached the aircraft and extinguished the fire on the #3 engine. Passengers and crew evacuated the aircraft with no injuries, evacuation checklist completed. Supplemental information from acn 154267. I believe the first indication of engine failure was the 'engine failure lights' on the captain's and first officer's glare shields. I terminated the dump and noticed out of the corner of my eye that the #3 start valve open light was on steady. I informed the pilots. On final approach, an aircraft waiting for takeoff transmitted that we had alot of smoke coming from #3 but he saw no flames. Supplemental information from acn 153837. Passengers evacuated out left forward and aft slides and a couple of people went out over wing exit. No panic or confusion with passenger's.

WAS ADVISED BY EITHER FSS OR TAXIING ACFT #3 ENG WAS STILL ON FIRE. TOLD FSS TO HAVE EQUIP MEET THE ACFT AFTER LNDG AND THAT I PLANNED TO EVACUATE THE ACFT ON RWY. ADVISED F/A'S ND PAXS ON P/A TO PLAN EVACUATION AFTER WE STOPPED ACFT. ON THE RWY SINCE BY THAT TIME WE WERE ON SHORT FINAL. LANDED ACFT AND AFTER IT WAS STOPPED I CALLED FOR EVACUATION ON THE L SIDE ONLY. THE FIRE DEPT IMMEDIATELY REACHED THE ACFT AND EXTINGUISHED THE FIRE ON THE #3 ENG. PAXS AND CREW EVACUATED THE ACFT WITH NO INJURIES, EVACUATION CHKLIST COMPLETED. SUPPLEMENTAL INFO FROM ACN 154267.
