Narrative:

During an approach brief for an approach to roa on ldg runway 6, I was avoiding WX that was on the approach path, tuning and identing the radios, while the first officer was giving the brief. The missed approach was to climb to 1800', then a climbing right turn to 4000' outbnd via odr VOR R127 degree to manat intersection and hold. I misunderstood and thought it said to go to the VOR (which is on the field) an outbnd on the right 127 degree. We did miss the approach. By the time we had the flaps set and the gear up we were at 1800' for our right turn climbing to 4000' as per the procedure. I was switching the frequency, dialing the HSI to the VOR and turning the heading bug. I told the first officer to bring her right on around. He, in his own mind, questioned the turn because I was turning the bug past the right. Although while doing as I instructed him to do he did not question me. Turning through about 220 degree, the controller did, by asking if the tower had given a heading, I said neg, he asked what were we turning to? I said the VOR, he said we were supposed to fly the R127 degree. I said yes sir that is correct. (At that time I was aware of my mistake). He gave us a heading to turn left to 220 degree and asked our intentions. We stated we wanted vectors for another approach. We then proceeded, landed, and nothing more was said. I am a firm believer in CRM and try to promote open communication with my crew. I feel sure that my first officer will not hesitate to bring to the captain's attention a question in his mind. As for myself, I'm sure I'll stop long enough to get the plan firmly in my mind.

Title: ACR MLG FAILED TO FOLLOW MISSED APCH PROC FOLLOWING A GAR AT ROA.

Narrative: DURING AN APCH BRIEF FOR AN APCH TO ROA ON LDG RWY 6, I WAS AVOIDING WX THAT WAS ON THE APCH PATH, TUNING AND IDENTING THE RADIOS, WHILE THE F/O WAS GIVING THE BRIEF. THE MISSED APCH WAS TO CLB TO 1800', THEN A CLBING R TURN TO 4000' OUTBND VIA ODR VOR R127 DEG TO MANAT INTXN AND HOLD. I MISUNDERSTOOD AND THOUGHT IT SAID TO GO TO THE VOR (WHICH IS ON THE FIELD) AN OUTBND ON THE R 127 DEG. WE DID MISS THE APCH. BY THE TIME WE HAD THE FLAPS SET AND THE GEAR UP WE WERE AT 1800' FOR OUR R TURN CLBING TO 4000' AS PER THE PROC. I WAS SWITCHING THE FREQ, DIALING THE HSI TO THE VOR AND TURNING THE HDG BUG. I TOLD THE F/O TO BRING HER R ON AROUND. HE, IN HIS OWN MIND, QUESTIONED THE TURN BECAUSE I WAS TURNING THE BUG PAST THE R. ALTHOUGH WHILE DOING AS I INSTRUCTED HIM TO DO HE DID NOT QUESTION ME. TURNING THROUGH ABOUT 220 DEG, THE CTLR DID, BY ASKING IF THE TWR HAD GIVEN A HDG, I SAID NEG, HE ASKED WHAT WERE WE TURNING TO? I SAID THE VOR, HE SAID WE WERE SUPPOSED TO FLY THE R127 DEG. I SAID YES SIR THAT IS CORRECT. (AT THAT TIME I WAS AWARE OF MY MISTAKE). HE GAVE US A HDG TO TURN L TO 220 DEG AND ASKED OUR INTENTIONS. WE STATED WE WANTED VECTORS FOR ANOTHER APCH. WE THEN PROCEEDED, LANDED, AND NOTHING MORE WAS SAID. I AM A FIRM BELIEVER IN CRM AND TRY TO PROMOTE OPEN COM WITH MY CREW. I FEEL SURE THAT MY F/O WILL NOT HESITATE TO BRING TO THE CAPT'S ATTN A QUESTION IN HIS MIND. AS FOR MYSELF, I'M SURE I'LL STOP LONG ENOUGH TO GET THE PLAN FIRMLY IN MY MIND.
