Narrative:

Flight ont-slc 6/90 departure time xx:50Z. Called for clearance at xa:35 Z, advised clearance on request. Called back at xz:45Z, advised clearance not filed. Operations failed to file clearance. Filed our own clearance with riverside flight service xo:50Z. Received pomona 4 as filed flight plan route/runway 26R. Pushed back for taxi at xy:10Z. During taxi tower changed runway to 8R. I requested new clearance. Tower advised us to turn right after takeoff to a heading of 110 degrees and maintain 9000', cleared us for takeoff. After contacting ont departure control, they advised us to turn right to 255 degrees, climb and maintain 14000'. While proceeding with ont, tower and ont departures instructions for heading and altitude, we assumed the pomona 4 departure was cancelled. We were never instructed to resume the pomona 4 departure. At 6500', we were told to comply with all restrictions. At 8500', we were told to descend to 8000' until the pom VOR. We made an immediate descent to 8000'. Operations were normal from that point on. The problem arose when the tower and departure control assumed we would continue the pom 4 and we assumed the pom 4 was cancelled. Contributing factors. Late clearance, runway change during taxi, 2 clearance changes during takeoff and climb. Human factor. Lack of clear communication and understanding between us, tower, departure control and center. Factors affecting quality of human performance. Communication, too many changes in a short period of time through too many facs (tower, departure control, center). We were well aware of the climb restrictions at pom. It was well briefed before takeoff and we even talked about the restrictions inbound the night before.

THE PROB AROSE WHEN THE TWR AND DEP CTL ASSUMED WE WOULD CONTINUE THE POM 4 AND WE ASSUMED THE POM 4 WAS CANCELLED. CONTRIBUTING FACTORS. LATE CLRNC, RWY CHANGE DURING TAXI, 2 CLRNC CHANGES DURING TKOF AND CLB. HUMAN FACTOR. LACK OF CLR COM AND UNDERSTANDING BTWN US, TWR, DEP CTL AND CTR. FACTORS AFFECTING QUALITY OF HUMAN PERFORMANCE. COM, TOO MANY CHANGES IN A SHORT PERIOD OF TIME THROUGH TOO MANY FACS (TWR, DEP CTL, CTR). WE WERE WELL AWARE OF THE CLB RESTRICTIONS AT POM. IT WAS WELL BRIEFED BEFORE TKOF AND WE EVEN TALKED ABOUT THE RESTRICTIONS INBND THE NIGHT BEFORE.
