Narrative:

Medium large transport X came on frequency. Radio controller had no knowledge of medium large transport X's location. Radar controller got medium large transport X's position, located departure strip and attempted to start a track on medium large transport X. At that time manual controller pointed to medium large transport X's limited data block and said start a track. Radar controller's attention was diverted to another area of radar pvd. Several mins after the conflict alert was activated between medium large transport X and medium large transport Y at FL330. At that time radar controller asked the manual controller where the medium large transport X had come from as the initial action to start a track on medium large transport X had failed. Medium large transport X had been leveled at FL190 due to over traffic at FL200. Medium large transport Z also on the same route as medium large transport X and was in front of medium large transport X at FL190. The conflict alert was activated between medium large transport Z and medium large transport X. Medium large transport X was vectored 40 degree to the right and descended to FL180.

Title: ARTCC RADAR CTLR FAILED TO PROVIDE STANDARD IFR SEPARATION BETWEEN TWO ACFT UNDER HIS CTL.

Narrative: MLG X CAME ON FREQ. RADIO CTLR HAD NO KNOWLEDGE OF MLG X'S LOCATION. RADAR CTLR GOT MLG X'S POSITION, LOCATED DEP STRIP AND ATTEMPTED TO START A TRACK ON MLG X. AT THAT TIME MANUAL CTLR POINTED TO MLG X'S LIMITED DATA BLOCK AND SAID START A TRACK. RADAR CTLR'S ATTN WAS DIVERTED TO ANOTHER AREA OF RADAR PVD. SEVERAL MINS AFTER THE CONFLICT ALERT WAS ACTIVATED BETWEEN MLG X AND MLG Y AT FL330. AT THAT TIME RADAR CTLR ASKED THE MANUAL CTLR WHERE THE MLG X HAD COME FROM AS THE INITIAL ACTION TO START A TRACK ON MLG X HAD FAILED. MLG X HAD BEEN LEVELED AT FL190 DUE TO OVER TFC AT FL200. MLG Z ALSO ON THE SAME ROUTE AS MLG X AND WAS IN FRONT OF MLG X AT FL190. THE CONFLICT ALERT WAS ACTIVATED BETWEEN MLG Z AND MLG X. MLG X WAS VECTORED 40 DEG TO THE RIGHT AND DESCENDED TO FL180.
