Narrative:

Approach and departure clrncs in the santo domingo area are a serious problem due to an abundance of fixes with K letters. This is compounded by language differences with most arriving/departing flts. A continuing discussion occurred between controller and inbound flight relative to routing. Clearance limit and routing were copied and read back several times and still resulted in disagreement with ground controller. All 3 crew members were used in the copy/readback phase. There needs to be a revision of intersection names with 'K' (spelling or sounding) to eliminate this problem in the santo domingo area.
