Following mels (deferred) in maintenance logbook: chapter 78-1; left thrust rev inop (inoperative). Chapter 29-2; center system electric pumps (# 2). Chapter 36-2; engine hi stage bleed valves/high pressure shutoff valve systems.

Title: A Captain and First Officer report about aborts; diversions; emergencies; numerous MEL deferrals and maintenance procedures that were not properly applied on a B767-300ER to Lock-out (pin); a left engine High Pressure Shut-off (HPSOV) valve. After the deferral; in flight EICAS messages included: AIR EMERGENCY ENG HPSOV-L with BLEED light illuminated. Medical emergency also declared.
