Narrative:

I was working east; satellite; sawgy; and west. An air carrier departed off of runway 25 at jax. I radar identified them; climbed them to 15;000 and turned them on initial contact. I had a departure off of crg and an overflight that I changed their routing and made an amendment in the flight data input/output (FIDO). The air carrier called approximately 7;000 ft and said he had traffic in front of them about 200 ft. I did not notice the target until he called and then I noticed a transponder with no mode-C. I told them I was showing a target with no mode-C. They said they were going to report a near miss. I let the supervisor know and they reviewed the tapes and voice recorder and made they necessary phone calls. The pilot asked if we needed to be talking to that aircraft at that altitude. I told them no. Recommendation; work to improve my scan. Ask for an overhead when it picks up. Have data input any changes into the FIDO.

Title: JAX Controller described a NMAC involving a departing Air Carrier and a VFR aircraft transitioning the area with no Mode-C; the reporter indicating the subject target was not observed prior to the event.
