Narrative:

IFR helicopter; from local hospital to another hospital. Was descended to 2000 feet on a vector to the IAF by approach then transferred to another sector. That controller asked if I was on the approach; they are not familiar with approaches to the local hospitals as they should be. I said no; I was told direct to the IAF and cleared for the approach. The controller told me I was leaving a 2000 MSA sector for a 2400 MSA sector. His voice and demeanor changed; I was given a turn and a climb. Approach clearance was not cancelled. I finally broke out; cancelled IFR; and proceeded VFR. I called the TRACON to inquire about the controllers anxiety. He said the issue was between controllers and that the helicopter only approach plates don't have enough geographic definition.
