Narrative:

I; the captain; was flying the aircraft. This was the third and final leg with the same aircraft and crew. On approach to localizer runway xxl. When selecting flaps from 1 to 5; EICAS message 'le flap asym' and 'leading edge' light illuminated. Broke off the approach; requested vectors to a holding fix to accomplish the QRH checklist. Maintained configuration and speed of flaps 5 and 187 KTS. Initially climbed to 3500 ft MSL and climbed to 4000 ft MSL to hold on the ILS runway yy. Accomplished the QRH 'le slat asym' checklist. Configuration was 20 flaps extended by altn te flaps with vref 30 to 30 (157 KTS). Declared an emergency at about XA30Z since there was a mechanical failure of the flap/slat system. Received vectors to ILS runway yy and landed at vref to 157 KTS. Fire rescue did not observe anything unusual and I taxied to the gate; leaving the flap configuration per maintenance request. This was repeat item of only two days prior.
