Narrative:

I climbed an aircraft to 6000' and did not pass the information to the adjacent approach control facility. While being relieved from the radar position, I forgot to brief the next controller adequately about the information. The aircraft was then transferred to the next approach facility at an altitude different than what he was supposed to be. This problem could have been corrected either by passing the information myself, or by telling the next controller to on the briefing. Callback conversation with reporter revealed the following: strips are generated by ARTCC and passed to both facs involved. GGG does not have fdep so any changes made while working shv must be voice coordination according to LOA. Reporter had climbed the small aircraft to 6000', marked the strip, but did not pass the information to GGG. Controller was relieved for a break and forgot to brief his relief that the information was not passed. GGG had a target on handoff, but mode C did not agree with strip information. Minor system deviation.

Title: SMA ENTERED ADJACENT FAC'S AIRSPACE AT AN ALT OTHER THAN SHOWN ON FLT PROGRESS STRIP. CTLR FORGOT TO COORDINATE.

Narrative: I CLBED AN ACFT TO 6000' AND DID NOT PASS THE INFO TO THE ADJACENT APCH CTL FAC. WHILE BEING RELIEVED FROM THE RADAR POS, I FORGOT TO BRIEF THE NEXT CTLR ADEQUATELY ABOUT THE INFO. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING: STRIPS ARE GENERATED BY ARTCC AND PASSED TO BOTH FACS INVOLVED. GGG DOES NOT HAVE FDEP SO ANY CHANGES MADE WHILE WORKING SHV MUST BE VOICE COORD ACCORDING TO LOA. RPTR HAD CLBED THE SMA TO 6000', MARKED THE STRIP, BUT DID NOT PASS THE INFO TO GGG. CTLR WAS RELIEVED FOR A BREAK AND FORGOT TO BRIEF HIS RELIEF THAT THE INFO WAS NOT PASSED. GGG HAD A TARGET ON HDOF, BUT MODE C DID NOT AGREE WITH STRIP INFO. MINOR SYS DEVIATION.
