Narrative:
Aircraft X on downwind at 12000 ft. Aircraft Y inbound on QUINN2 arrival. Radar controller cleared aircraft Y for approach to cross plage at 11000 ft. Aircraft X descended to 11000 ft and then turned base in front of aircraft Y to a 070 degree heading. Aircraft X turned to a 010 degree heading and told to join the localizer runway 34R. Aircraft Y turned to a 030 degree heading and told to maintain 11000 ft for traffic. Separation lost. Aircraft X cleared for ILS runway 34R approach. Aircraft Y turned to 310 degree heading behind aircraft X. Aircraft Y cleared for ILS runway 34R approach.

Title: S56 HDOF CTLR WITNESSED OPERROR AT 11000 WHEN CTLR FAILED TO ENSURE SEPARATION BETWEEN TWO ARR ACFT; ONE APPARENTLY ON A VISUAL APCH.
Narrative: ACFT X ON DOWNWIND AT 12000 FT. ACFT Y INBOUND ON QUINN2 ARR. RADAR CTLR CLRED ACFT Y FOR APCH TO CROSS PLAGE AT 11000 FT. ACFT X DSNDED TO 11000 FT AND THEN TURNED BASE IN FRONT OF ACFT Y TO A 070 DEG HDG. ACFT X TURNED TO A 010 DEG HDG AND TOLD TO JOIN THE LOC RWY 34R. ACFT Y TURNED TO A 030 DEG HDG AND TOLD TO MAINTAIN 11000 FT FOR TFC. SEPARATION LOST. ACFT X CLRED FOR ILS RWY 34R APCH. ACFT Y TURNED TO 310 DEG HDG BEHIND ACFT X. ACFT Y CLRED FOR ILS RWY 34R APCH.
