Narrative:

I worked flight attendant #5 and failed to properly complete a portion of my safety check. On boarding; I turned to my manual for my preflight inspection. I completed the check for the first 6 bulleted items. To complete the final item -- door 3R crew rest entry enclosure/lower lobe. While I was beginning the 'llcr' check; a pilot who had never seen it came and chatted at the door and looked down the hatch. Once he left; I didn't follow through to complete the check using the preflight equipment inspections pages. There is no explanation for why I did not follow through with the rest of the check. I did ensure the hatch was closed during takeoff and landing. Mid-flight; beginning the third break; a flight attendant (purser?) discovered that the 'arm extinguisher' and 'air flow' lights were illuminated. At this time I walked by; was asked if my manual was available. The cockpit was notified and we reviewed our manual for required procedures. I realized at this point I did not fully complete my preflight inspection. Next we pressed the 'arm extinguisher' button and closed and opened the hatch. Airflow was re-established. The crew did not know if the arm button was inadvertently hit as flight attendants entered or exited the lower lobe during the breaks or if it was on from the beginning as I did not check the asp or llcr.

Title: CABIN ATTENDANT FAILS TO COMPLETE PREFLT INSPECTION OF LOWER LOBE REST AREA OF B777. INFLT SHE NOTES FAILURE TO ARM EXTINGUISHER AND ESTABLISH AIR FLOW TO THE REST AREA.

Narrative: I WORKED FLT ATTENDANT #5 AND FAILED TO PROPERLY COMPLETE A PORTION OF MY SAFETY CHK. ON BOARDING; I TURNED TO MY MANUAL FOR MY PREFLT INSPECTION. I COMPLETED THE CHK FOR THE FIRST 6 BULLETED ITEMS. TO COMPLETE THE FINAL ITEM -- DOOR 3R CREW REST ENTRY ENCLOSURE/LOWER LOBE. WHILE I WAS BEGINNING THE 'LLCR' CHK; A PLT WHO HAD NEVER SEEN IT CAME AND CHATTED AT THE DOOR AND LOOKED DOWN THE HATCH. ONCE HE LEFT; I DIDN'T FOLLOW THROUGH TO COMPLETE THE CHK USING THE PREFLT EQUIP INSPECTIONS PAGES. THERE IS NO EXPLANATION FOR WHY I DID NOT FOLLOW THROUGH WITH THE REST OF THE CHK. I DID ENSURE THE HATCH WAS CLOSED DURING TKOF AND LNDG. MID-FLT; BEGINNING THE THIRD BREAK; A FLT ATTENDANT (PURSER?) DISCOVERED THAT THE 'ARM EXTINGUISHER' AND 'AIR FLOW' LIGHTS WERE ILLUMINATED. AT THIS TIME I WALKED BY; WAS ASKED IF MY MANUAL WAS AVAILABLE. THE COCKPIT WAS NOTIFIED AND WE REVIEWED OUR MANUAL FOR REQUIRED PROCS. I REALIZED AT THIS POINT I DID NOT FULLY COMPLETE MY PREFLT INSPECTION. NEXT WE PRESSED THE 'ARM EXTINGUISHER' BUTTON AND CLOSED AND OPENED THE HATCH. AIRFLOW WAS RE-ESTABLISHED. THE CREW DID NOT KNOW IF THE ARM BUTTON WAS INADVERTENTLY HIT AS FLT ATTENDANTS ENTERED OR EXITED THE LOWER LOBE DURING THE BREAKS OR IF IT WAS ON FROM THE BEGINNING AS I DID NOT CHK THE ASP OR LLCR.
