While on watch as controller in charge; supervisor performed OJT instruction on ground control with developmental ATC specialist. Aircraft taxied to runway 11 for departure. Aircraft on IFR flight plan. Ground control failed to coordination with local control IFR status. Aircraft calls local control for departure. Local control departed aircraft; assumed VFR; aircraft proceeded northbound IFR without coordination approach. Approach control A90 via shout line requested information from tower regarding aircraft on IFR transponder code; aircraft squawking wrong code for aircraft identification. Code corrected via local control frequency; approach control A90 issued heading 040 altitude climb 3000 ft via shout line to local control; assume aircraft below MVA for area. Aircraft frequency change A90. Training on ground control term; supervisor aware of situation informs controller in charge no loss of separation. Controller in charge called for meeting with atm and supervisor regarding other matters. No paper on error; no direction given with regard to error. ATC specialist concerned situation was operational deviation; afraid to question situation. Contributing factors: busy traffic on local control; attention focus on local control traffic; training ground control OJT instruction not attentive to developmental ATC specialist; controller in charge focus on cc duties and correction of local control phraseology and traffic calls.
