Air carrier X on a visual approach to san jose airport reported near miss about 15 mi out. At that time I noticed primary target only (no mode C). Traffic was not issued.

Title: NCT CTLR DESCRIBED CONFLICT AT 5500 BETWEEN SJC ACR ARR AND PRIMARY TARGET THAT WAS NOT ISSUED.
Narrative: ACR X ON A VISUAL APCH TO SAN JOSE ARPT RPTED NEAR MISS ABOUT 15 MI OUT. AT THAT TIME I NOTICED PRIMARY TARGET ONLY (NO MODE C). TFC WAS NOT ISSUED.
