Narrative:

Left-hand aileron hinge bushings worn. Aileron was removed and bushings replaced. Aileron damper removed and replaced due to shifted bearing during removal. Damper was installed by maintenance but damper to wing attach bolt worked loose causing minor damage. Back side of 'write up' shows my inspection stamp. I feel with 31 yrs maintenance experience and 6 months since the installation of the damper; it would be most unusual to miss the safety of the bolt in question. It would have been nice to inspect the condition of the bolt that was found unsecured by ZZZ maintenance.

Title: AN EMB-135KL (ERJ-140LR) AILERON DAMPER TO WING ATTACH BOLT WORKED LOOSE CAUSING MINOR DAMAGE. BOLT NOT SAFETIED. AILERON BUSHINGS PREVIOUSLY REPLACED.

Narrative: L-HAND AILERON HINGE BUSHINGS WORN. AILERON WAS REMOVED AND BUSHINGS REPLACED. AILERON DAMPER REMOVED AND REPLACED DUE TO SHIFTED BEARING DURING REMOVAL. DAMPER WAS INSTALLED BY MAINT BUT DAMPER TO WING ATTACH BOLT WORKED LOOSE CAUSING MINOR DAMAGE. BACK SIDE OF 'WRITE UP' SHOWS MY INSPECTION STAMP. I FEEL WITH 31 YRS MAINT EXPERIENCE AND 6 MONTHS SINCE THE INSTALLATION OF THE DAMPER; IT WOULD BE MOST UNUSUAL TO MISS THE SAFETY OF THE BOLT IN QUESTION. IT WOULD HAVE BEEN NICE TO INSPECT THE CONDITION OF THE BOLT THAT WAS FOUND UNSECURED BY ZZZ MAINT.
