Narrative:

On logbook page X; items #26; #27; and #28 were deferred per MEL; entered in sheet; but aircraft placard; air carrier form; was not placed in front of logbook. This was brought to air carrier's attention and placards were filled out and placed in logbook. I am filling out this form to report it. The placards were placed in the logbook this morning and aircraft is ok for continued service. The only reason I can give for not putting the placards in the logbook was due to me being distraction and interrupted. Everything was done except the placing of the placard.

Narrative: ON LOGBOOK PAGE X; ITEMS #26; #27; AND #28 WERE DEFERRED PER MEL; ENTERED IN SHEET; BUT ACFT PLACARD; ACR FORM; WAS NOT PLACED IN FRONT OF LOGBOOK. THIS WAS BROUGHT TO ACR'S ATTN AND PLACARDS WERE FILLED OUT AND PLACED IN LOGBOOK. I AM FILLING OUT THIS FORM TO RPT IT. THE PLACARDS WERE PLACED IN THE LOGBOOK THIS MORNING AND ACFT IS OK FOR CONTINUED SVC. THE ONLY REASON I CAN GIVE FOR NOT PUTTING THE PLACARDS IN THE LOGBOOK WAS DUE TO ME BEING DISTR AND INTERRUPTED. EVERYTHING WAS DONE EXCEPT THE PLACING OF THE PLACARD.
