Narrative:
We simply misunderstood the SID. I leveled at 1500 ft and was waiting for 4.5 DME from teb to make the turn and climb to 2000 ft. I was supposed to make the turn upon reaching 1500 ft then at 4.5 DME climb to 2000 ft. The description of the departure is broken up into 2 separate boxes which added to the misunderstanding.

Title: A C650 DEPARTING TEB ON THE TEB DEP FAILED TO TURN AS CHARTED AT 1500 FT; AT LEAST IN PART BECAUSE OF CHART CONFUSION.
Narrative: WE SIMPLY MISUNDERSTOOD THE SID. I LEVELED AT 1500 FT AND WAS WAITING FOR 4.5 DME FROM TEB TO MAKE THE TURN AND CLB TO 2000 FT. I WAS SUPPOSED TO MAKE THE TURN UPON REACHING 1500 FT THEN AT 4.5 DME CLB TO 2000 FT. THE DESCRIPTION OF THE DEP IS BROKEN UP INTO 2 SEPARATE BOXES WHICH ADDED TO THE MISUNDERSTANDING.
