Narrative:

Passing through 17000 ft; we received a call from the flight attendant. He said he had hit his head and was bleeding. I took a look at him. He had a gash just over the bridge of his nose and bleeding profusely. I asked if he wanted assistance and he said yes. I declared a medical emergency. We went back into ZZZ. Medical personnel met us at the gate; treated the flight attendant and took him away on a gurney. The first officer flew the aircraft while I coordination with operations dispatch and ATC.

Title: EMB135 CABIN CREW MEMBER RECEIVED AN INJURY DURING DEP; EMER DECLARED AND ACFT RETURNED FOR LNDG.

Narrative: PASSING THROUGH 17000 FT; WE RECEIVED A CALL FROM THE FLT ATTENDANT. HE SAID HE HAD HIT HIS HEAD AND WAS BLEEDING. I TOOK A LOOK AT HIM. HE HAD A GASH JUST OVER THE BRIDGE OF HIS NOSE AND BLEEDING PROFUSELY. I ASKED IF HE WANTED ASSISTANCE AND HE SAID YES. I DECLARED A MEDICAL EMER. WE WENT BACK INTO ZZZ. MEDICAL PERSONNEL MET US AT THE GATE; TREATED THE FLT ATTENDANT AND TOOK HIM AWAY ON A GURNEY. THE FO FLEW THE ACFT WHILE I COORD WITH OPS DISPATCH AND ATC.
