Narrative:

Passenger seated in the aft cabin was very ill. Passenger 79 yrs old was found vomiting in her sleep; incoherent to her surroundings. The flight attendants were in the process of reviving passenger and had alerted first officer about 70 mins north of ZZZ. First officer commanded the relief pilot to fly the airplane and flight attendants to awaken and notify me of our status. When I reported to the flight deck; first officer was in the process of paging the cabin for a physician on board with dispatch and physician on call on a 3-WAY call over satcom. At this time a physician on board came forward as noted by flight attendants. Reports at this time from the flight attendants were that the physician on board was experiencing difficulty reviving passenger. The physician on board confirmed that passenger would most likely continue to vomit due to some gastro intestinal disorder that a traveling companion of passenger confirmed she was taking some type of medication for. We were also concerned with this additional information that passenger would become rapidly dehydrated. At this point we notified dispatch of our status and jointly concluded we would declare a medical emergency and divert. Declared a medical emergency with control. Reviewed overweight landing criteria and procedures. Briefed our arrival and approach. Landing flaps 25 degrees to a smooth landing 3900 pounds overweight. Medical personnel boarded the aircraft and assisted in the deplaning of passenger.
