Narrative:

Approximately 30 mins prior to landing; l-hand slide window exit slide deployed in mid air. Cockpit was informed once problem was idented; and necessary procedures were taken in order for landing. Callback conversation with reporter revealed the following information: reporter stated that initially the crew and flight attendants had no idea what caused the large airframe jolt and vibration. The flight attendants in the aft cabin near the door thought a bomb had exploded but saw no smoke. Not until some time later did anyone look out the window to see the slide in the wing's slipstream. The reporter stated when the aircraft landed; very little of the slide remained and the fuselage slide door was damaged. Reporter stated the door indicating system had been labeled faulty by maintenance prior to the flight.

Title: A B757 EMER SLIDE AT DOOR 3L DEPLOYED INFLT AFTER THE DOOR INDICATING SYS WAS LABELED FAULTY BY MAINT PRIOR TO FLT.

Narrative: APPROX 30 MINS PRIOR TO LNDG; L-HAND SLIDE WINDOW EXIT SLIDE DEPLOYED IN MID AIR. COCKPIT WAS INFORMED ONCE PROB WAS IDENTED; AND NECESSARY PROCS WERE TAKEN IN ORDER FOR LNDG. CALLBACK CONVERSATION WITH RPTR REVEALED THE FOLLOWING INFO: RPTR STATED THAT INITIALLY THE CREW AND FLT ATTENDANTS HAD NO IDEA WHAT CAUSED THE LARGE AIRFRAME JOLT AND VIBRATION. THE FLT ATTENDANTS IN THE AFT CABIN NEAR THE DOOR THOUGHT A BOMB HAD EXPLODED BUT SAW NO SMOKE. NOT UNTIL SOME TIME LATER DID ANYONE LOOK OUT THE WINDOW TO SEE THE SLIDE IN THE WING'S SLIPSTREAM. THE RPTR STATED WHEN THE ACFT LANDED; VERY LITTLE OF THE SLIDE REMAINED AND THE FUSELAGE SLIDE DOOR WAS DAMAGED. RPTR STATED THE DOOR INDICATING SYS HAD BEEN LABELED FAULTY BY MAINT PRIOR TO THE FLT.
