Narrative:
A standardization to change uret to strips must be established. I took over a radar position during this transition; and had an operational error within 2 mins. No strips; no uret; just 'wingin' it' the best I could. I have repeatedly complained of the lack of organization during this transition. It wasn't clear who I had communication with and whom I didn't; and in the confusion I thought I was using 'altitude leaving' to separate said aircraft.
