Narrative:

DC9 and BE20 were being vectored to final. BE20 was not issued a turn to the downwind and overflew ind and became a conflict with DC9 on the opposite downwind. As separation was being lost; BE20 was vectored away from DC9. The conflict alert did not sound until after separation had been regained. Contributing factors: volume of traffic; frequency congestion; and overall workload.

Title: IND TRACON CTLR EXPERIENCED OPERROR WHEN FAILING TO ISSUE DOWNWIND TURN TO ARR ACFT.

Narrative: DC9 AND BE20 WERE BEING VECTORED TO FINAL. BE20 WAS NOT ISSUED A TURN TO THE DOWNWIND AND OVERFLEW IND AND BECAME A CONFLICT WITH DC9 ON THE OPPOSITE DOWNWIND. AS SEPARATION WAS BEING LOST; BE20 WAS VECTORED AWAY FROM DC9. THE CONFLICT ALERT DID NOT SOUND UNTIL AFTER SEPARATION HAD BEEN REGAINED. CONTRIBUTING FACTORS: VOLUME OF TFC; FREQ CONGESTION; AND OVERALL WORKLOAD.
