Narrative:

Assigned to a double crew to fly B747 for cargo flight abcc and abcd on jun/fri/05. The aircraft had the thrust reverser un-stow indicating system deferred per MEL 78-3-1A. The aircraft also had the autospoilers system deferred per MEL 27-9. Upon landing in incheon (icn); the #1 reverser did not deploy. Under 78-3-1A; one may be inoperative (and the reverser used) provided that; before each departure; it is verified that: 1) no damage exists which would impair structural or operational integrity of the system or components; 2) reverser sleeves are fully and properly stowed; 3) reverse thrust (full reverse) position indicating system; and 4) thrust reverser armed indicating system (jt-90-70A) operates normally. Under 27-9; may be inoperative provided it is deactivated and all fan reversers are operable and the antiskid system is operative. Note: reference 78-1; autospoilers must operate normally if reverser is inoperative. It was discovered that the #1 thrust reverser was inoperative due to being locked closed. It had been locked closed per MEL 78-3-1A; which did not call for the reverser to be locked out. MEL 78-3-1B calls for the reverser to be locked out per MEL 78-1.

Title: A B747 WAS DISPATCHED WITH 1 THRUST REVERSER LOCKED CLOSED AND THE AUTOSPOILERS INOP BOTH DEFERRED PER THE MEL. REVERSER COULD NOT BE LOCKED CLOSED TO MEET MEL LIMITS.

THE ACFT ALSO HAD THE AUTOSPOILERS SYS DEFERRED PER MEL 27-9. UPON LNDG IN INCHEON (ICN); THE #1 REVERSER DID NOT DEPLOY.
