Narrative:

We were in the before starting engines checklist when the agent stuck his head in the cockpit and asked if we were ready to go. We replied yes. We finished the checklist up to the jp's. The #1 flight attendant gave us her cabin ready report and closed the cockpit door. The ground crew checked in all clear below; ready for brake release. Captain said stand-by. We finished the before starting engines checklist including confirming all door light extinguished; to packs off. I called clearance for push then switched to ground. Ground cleared us to push. Captain told ground crew; brakes released; cleared for push. The push crew began the push. I heard a loud scrape and aircraft stopped. Unknown to us the agent had signaled the #1 flight attendant to open the forward entry door without notifying us. The ground crew did not see the agent open the door. We did not see the doors lights on the overhead panel come on after it was verified out; nor the EICAS message. Damage was done to the forward door escape slide assembly. Callback conversation with reporter revealed the following information: reporter stated this incident was a combination of errors. The agent did not attempt to get the crew's attention; the flight attendants did not notify the cockpit; the flight crew did not see the door light illuminate after it was out; and the push crew did not see the jetbridge move to the aircraft.

THE AGENT DID NOT ATTEMPT TO GET THE CREW'S ATTN; THE FLT ATTENDANTS DID NOT NOTIFY THE COCKPIT; THE FLT CREW DID NOT SEE THE DOOR LIGHT ILLUMINATE AFTER IT WAS OUT; AND THE PUSH CREW DID NOT SEE THE JETBRIDGE MOVE TO THE ACFT.
