Narrative:

Aircraft was given an approach clearance 8 mi northeast of las vegas airport. Aircraft was already slowed to 170 KTS for sequence. The instructions were for the aircraft to turn the final at the stratosphere for a visual approach to runway 19L. The pilot elected to widen out his base for the turn into the airport causing an airspace violation with nellis approach. If the pilot were to have followed the instructions given (which he read back in the clearance); this situation would not have occurred. One contributing factor with this situation was that the flow rate into las was set too high for runway acceptance. This causes us to offload to a runway which has high terrain causing us to keep the airplane high for the approach. The agency seems to have changed modes from one of safety to one of volume. This has caused numerous safety issues at las. I wish that the agency would put safety first and reduce the volume to acceptable levels.

Title: FAILURE OF THE FLT CREW TO COMPLY WITH CLRNC; COMPOUNDED BY HVY ARR VOLUME; RESULTED IN THE ACFT ENTERING ADJACENT MIL AIRSPACE WITHOUT AUTH.

Narrative: ACFT WAS GIVEN AN APCH CLRNC 8 MI NE OF LAS VEGAS ARPT. ACFT WAS ALREADY SLOWED TO 170 KTS FOR SEQUENCE. THE INSTRUCTIONS WERE FOR THE ACFT TO TURN THE FINAL AT THE STRATOSPHERE FOR A VISUAL APCH TO RWY 19L. THE PLT ELECTED TO WIDEN OUT HIS BASE FOR THE TURN INTO THE ARPT CAUSING AN AIRSPACE VIOLATION WITH NELLIS APCH. IF THE PLT WERE TO HAVE FOLLOWED THE INSTRUCTIONS GIVEN (WHICH HE READ BACK IN THE CLRNC); THIS SIT WOULD NOT HAVE OCCURRED. ONE CONTRIBUTING FACTOR WITH THIS SIT WAS THAT THE FLOW RATE INTO LAS WAS SET TOO HIGH FOR RWY ACCEPTANCE. THIS CAUSES US TO OFFLOAD TO A RWY WHICH HAS HIGH TERRAIN CAUSING US TO KEEP THE AIRPLANE HIGH FOR THE APCH. THE AGENCY SEEMS TO HAVE CHANGED MODES FROM ONE OF SAFETY TO ONE OF VOLUME. THIS HAS CAUSED NUMEROUS SAFETY ISSUES AT LAS. I WISH THAT THE AGENCY WOULD PUT SAFETY FIRST AND REDUCE THE VOLUME TO ACCEPTABLE LEVELS.
