Narrative:

Just prior to closing the passenger door, the csr came up and asked if we would power away from the gate. Normally this is standard procedure. However, because another aircraft was still at the gate (due to a mechanical) it would be impossible to power away (because the aircraft was too close to us and the heavy weight of our aircraft we would strike adjacent aircraft). Csr seemed upset but said ok and left, closing door. Ramp came on headset and asked us to power away also and I refused, stating that I would need a pushback. Ramp procured a tug, connected it and asked if I would release brakes? I did. Meanwhile the first officer, contacted ZHN for clearance to sfo (it was late and lih tower was closed). After clearance given, center told us to call them ready to taxi. Meanwhile, the ramper was given the ok to push back after we'd cleared traffic via CTAF. I set brakes upon request and had begun starting engines per ramp's clearance to do so. As the first officer began starting the 2ND engine, the ramper asked us to defer start for now. Ramper said he was disconnecting and said 'bye.' I got him to answer to find out whether we were ok to start the 2ND engine and asked him to use standard phraseology as I was uncomfortable with the way things were going. He replied, saying he was disconnected. I told him that he was cleared to disconnect and that I would watch for his salute. The ramper pulled the tug away and immediately the first officer called ZHN stating that we were ready to taxi. I told the first officer that I was not ready to taxi as I had not received a salute and release yet. Approximately 1 min later a voice down below connected to the headset said 'hello this is ramp' to which I replied go ahead. He then repeated the request to disconnect and I agreed. Finally a salute and release was received and we taxied out for departure. At this point the first officer seemed upset that I had mentioned that he needed to follow SOP. Nevertheless, on taxi-out towards the approach end of the runway for takeoff, he, upon finishing the before takeoff checklist went right into the final items checklist without my requesting it, even going so far as to perform my portion of the pt out of lih. He said that his job was to get this aircraft in the sky. I reminded him of SOP and the fact that the engines had not been sufficiently warmed up (it had been 3 mins). At this point he became extremely irate at me for questioning him. I listened, keeping my cool, but made sure that he understood the need to allow me to captain the aircraft and to slow down for safety's sake. We allowed the engines to warm completely and called out our intent to take the runway for departure and got underway. At top of climb he started lamenting a whole laundry list of items which I will save for professional standards, but suffice to say that he began a condescending conversation that I eventually had to put a stop to by asking him to just stick with essential communication. In conclusion, the lih operation became non-standard as a result of the delayed lax flight which prevented our pwring from the gate. At this point station personnel applied pressure to try and get me to attempt an unsafe (clearance wise) power away, which I refused. The first officer compounded matters by acting in a hurried and, oftentimes, subtly belligerent manner which he had used several times during the time we flew together. Calling him on it, he became verbally hostile. It is my observation that the hawaiian operation rushes to get us out of town so often that many flight crews have adopted a lax attitude about sops and are flying out of the islands in a very rushed, and possibly, forgetful manner.

Title: CAPT OF B757-200 RPTS COMPANY PERSONNEL FAILING TO FOLLOW SOPS.

Narrative: JUST PRIOR TO CLOSING THE PAX DOOR, THE CSR CAME UP AND ASKED IF WE WOULD PWR AWAY FROM THE GATE. NORMALLY THIS IS STANDARD PROC. HOWEVER, BECAUSE ANOTHER ACFT WAS STILL AT THE GATE (DUE TO A MECHANICAL) IT WOULD BE IMPOSSIBLE TO PWR AWAY (BECAUSE THE ACFT WAS TOO CLOSE TO US AND THE HVY WT OF OUR ACFT WE WOULD STRIKE ADJACENT ACFT). CSR SEEMED UPSET BUT SAID OK AND LEFT, CLOSING DOOR. RAMP CAME ON HEADSET AND ASKED US TO PWR AWAY ALSO AND I REFUSED, STATING THAT I WOULD NEED A PUSHBACK. RAMP PROCURED A TUG, CONNECTED IT AND ASKED IF I WOULD RELEASE BRAKES? I DID. MEANWHILE THE FO, CONTACTED ZHN FOR CLRNC TO SFO (IT WAS LATE AND LIH TWR WAS CLOSED). AFTER CLRNC GIVEN, CTR TOLD US TO CALL THEM READY TO TAXI. MEANWHILE, THE RAMPER WAS GIVEN THE OK TO PUSH BACK AFTER WE'D CLRED TFC VIA CTAF. I SET BRAKES UPON REQUEST AND HAD BEGUN STARTING ENGS PER RAMP'S CLRNC TO DO SO. AS THE FO BEGAN STARTING THE 2ND ENG, THE RAMPER ASKED US TO DEFER START FOR NOW. RAMPER SAID HE WAS DISCONNECTING AND SAID 'BYE.' I GOT HIM TO ANSWER TO FIND OUT WHETHER WE WERE OK TO START THE 2ND ENG AND ASKED HIM TO USE STANDARD PHRASEOLOGY AS I WAS UNCOMFORTABLE WITH THE WAY THINGS WERE GOING. HE REPLIED, SAYING HE WAS DISCONNECTED. I TOLD HIM THAT HE WAS CLRED TO DISCONNECT AND THAT I WOULD WATCH FOR HIS SALUTE. THE RAMPER PULLED THE TUG AWAY AND IMMEDIATELY THE FO CALLED ZHN STATING THAT WE WERE READY TO TAXI. I TOLD THE FO THAT I WAS NOT READY TO TAXI AS I HAD NOT RECEIVED A SALUTE AND RELEASE YET. APPROX 1 MIN LATER A VOICE DOWN BELOW CONNECTED TO THE HEADSET SAID 'HELLO THIS IS RAMP' TO WHICH I REPLIED GO AHEAD. HE THEN REPEATED THE REQUEST TO DISCONNECT AND I AGREED. FINALLY A SALUTE AND RELEASE WAS RECEIVED AND WE TAXIED OUT FOR DEP. AT THIS POINT THE FO SEEMED UPSET THAT I HAD MENTIONED THAT HE NEEDED TO FOLLOW SOP. NEVERTHELESS, ON TAXI-OUT TOWARDS THE APCH END OF THE RWY FOR TKOF, HE, UPON FINISHING THE BEFORE TKOF CHKLIST WENT RIGHT INTO THE FINAL ITEMS CHKLIST WITHOUT MY REQUESTING IT, EVEN GOING SO FAR AS TO PERFORM MY PORTION OF THE PT OUT OF LIH. HE SAID THAT HIS JOB WAS TO GET THIS ACFT IN THE SKY. I REMINDED HIM OF SOP AND THE FACT THAT THE ENGS HAD NOT BEEN SUFFICIENTLY WARMED UP (IT HAD BEEN 3 MINS). AT THIS POINT HE BECAME EXTREMELY IRATE AT ME FOR QUESTIONING HIM. I LISTENED, KEEPING MY COOL, BUT MADE SURE THAT HE UNDERSTOOD THE NEED TO ALLOW ME TO CAPT THE ACFT AND TO SLOW DOWN FOR SAFETY'S SAKE. WE ALLOWED THE ENGS TO WARM COMPLETELY AND CALLED OUT OUR INTENT TO TAKE THE RWY FOR DEP AND GOT UNDERWAY. AT TOP OF CLB HE STARTED LAMENTING A WHOLE LAUNDRY LIST OF ITEMS WHICH I WILL SAVE FOR PROFESSIONAL STANDARDS, BUT SUFFICE TO SAY THAT HE BEGAN A CONDESCENDING CONVERSATION THAT I EVENTUALLY HAD TO PUT A STOP TO BY ASKING HIM TO JUST STICK WITH ESSENTIAL COM. IN CONCLUSION, THE LIH OP BECAME NON-STANDARD AS A RESULT OF THE DELAYED LAX FLT WHICH PREVENTED OUR PWRING FROM THE GATE. AT THIS POINT STATION PERSONNEL APPLIED PRESSURE TO TRY AND GET ME TO ATTEMPT AN UNSAFE (CLRNC WISE) PWR AWAY, WHICH I REFUSED. THE FO COMPOUNDED MATTERS BY ACTING IN A HURRIED AND, OFTENTIMES, SUBTLY BELLIGERENT MANNER WHICH HE HAD USED SEVERAL TIMES DURING THE TIME WE FLEW TOGETHER. CALLING HIM ON IT, HE BECAME VERBALLY HOSTILE. IT IS MY OBSERVATION THAT THE HAWAIIAN OP RUSHES TO GET US OUT OF TOWN SO OFTEN THAT MANY FLT CREWS HAVE ADOPTED A LAX ATTITUDE ABOUT SOPS AND ARE FLYING OUT OF THE ISLANDS IN A VERY RUSHED, AND POSSIBLY, FORGETFUL MANNER.
