Aircraft X departed ZZZ without proper documentation of a no-fix/no-fly MEL item for a cockpit door problem. The item was worked on afternoon shift. The item was not repaired on night shift and the paperwork was not completed correctly prior to aircraft X departing ZZZ.

Narrative: ACFT X DEPARTED ZZZ WITHOUT PROPER DOCUMENTATION OF A NO-FIX/NO-FLY MEL ITEM FOR A COCKPIT DOOR PROB. THE ITEM WAS WORKED ON AFTERNOON SHIFT. THE ITEM WAS NOT REPAIRED ON NIGHT SHIFT AND THE PAPERWORK WAS NOT COMPLETED CORRECTLY PRIOR TO ACFT X DEPARTING ZZZ.
