Narrative:

Sse of haf at approximately 12000 ft in communication with ATC (on flight following), VFR -- maybe 9-10 NM from woodside VOR on about 295 degree radial. Was in cruise and at this same altitude and heading for at least 1 hour. Communication frequency 133.95, previous frequency 125.45 (oakland). B747 was observed banking to our left -- we were northbound. B747 was lower than we were when observed in the banking turn. B747 also contacted approach (on our same frequency 133.95) to report a TCASII alert and an near midair collision with a cessna (us). B747 requested 'forms' to report incident.

B747 WAS LOWER THAN WE WERE WHEN OBSERVED IN THE BANKING TURN. B747 ALSO CONTACTED APCH (ON OUR SAME FREQ 133.95) TO RPT A TCASII ALERT AND AN NMAC WITH A CESSNA (US). B747 REQUESTED 'FORMS' TO RPT INCIDENT.
