Narrative:

It was discovered that a single fuel xfeed switch was in the valve-open position. It is not known when this happened or even whether the crew inadvertently pressed the switch or whether it was missed on preflight. A fuel imbalance wing-to-wing had developed, which was considerably out of limits. Crew descended to lower altitude to increase total burn, and xfed to return fuel balance to within limits prior to landing, touching down with 500 pounds difference between tanks.

Title: FLT CREW OF B757 ENCOUNTERED WING TO WING FUEL IMBALANCE WHEN 1 OF 2 FUEL XFEED SWITCHES WAS LEFT IMPROPERLY IN THE OPEN POS.

Narrative: IT WAS DISCOVERED THAT A SINGLE FUEL XFEED SWITCH WAS IN THE VALVE-OPEN POS. IT IS NOT KNOWN WHEN THIS HAPPENED OR EVEN WHETHER THE CREW INADVERTENTLY PRESSED THE SWITCH OR WHETHER IT WAS MISSED ON PREFLT. A FUEL IMBALANCE WING-TO-WING HAD DEVELOPED, WHICH WAS CONSIDERABLY OUT OF LIMITS. CREW DSNDED TO LOWER ALT TO INCREASE TOTAL BURN, AND XFED TO RETURN FUEL BAL TO WITHIN LIMITS PRIOR TO LNDG, TOUCHING DOWN WITH 500 LBS DIFFERENCE BTWN TANKS.
