Narrative:
On jun/thu/03, I was given a deviation list by technician stating the left aft lavatory ashtray insert was missing. I had no knowledge at the time the deviation list was issued in error. I should have applied an MEL to the missing insert.

Title: A B757-200 WAS DISPATCHED IN NON COMPLIANCE WITH THE L AFT LAVATORY ASHTRAY INSERT MISSING AND DEFERRED WITH A DEV LIST REF.
Narrative: ON JUN/THU/03, I WAS GIVEN A DEV LIST BY TECHNICIAN STATING THE L AFT LAVATORY ASHTRAY INSERT WAS MISSING. I HAD NO KNOWLEDGE AT THE TIME THE DEV LIST WAS ISSUED IN ERROR. I SHOULD HAVE APPLIED AN MEL TO THE MISSING INSERT.
