Narrative:

Dispatch release error. I got a call from ZZZ operations agent advising that there was a delay on flight to ZZZ1 due to fueling. It turns out that they hadn't been shorted, but that the captain was adding 1000 pounds. This figure immediately rang a bell with me, and when I checked the dispatch release that I'd sent earlier, sure enough, I had failed to include the 1000 pound fuel required by the airworthiness directive on the -700, plus a corresponding remark. I re-ran the dispatch release to include these items, and revision-1 was sent at XH05. I later got a call from the captain upon arrival at ZZZ1 regarding the delay at ZZZ and my failing to have properly included the airworthiness directive fuel earlier, and I apologized for my error. It became evident to me from looking back at his times that he had not received the revision-1 that I'd sent earlier. My error notwithstanding, I suggested to the captain that a phone call could have been initiated before departure from ZZZ (when he first realized the airworthiness directive fuel had not been included upon his original dispatch release) and that we could have accomplished release changes verbally. He mentioned that he'd be filing a report, and I appreciate being invited in as well. Contributing to the situation was the fact that mdw had been below landing minimums between approximately XA00 and XG00 and that numerous irregular operations were in progress. When I'd taken the shift at XC00, I inherited 1 mdw diversion (in progress) eventually along with 4-5 other mdw flts that were being held at their departure points. 3 of these were ZZZ-mdw flts, and the overall number of phone/radio calls had been pretty heavy, especially when mdw came above minimums and I was trying to get everyone else going at roughly the same time as this flight's departure. Supplemental information from acn 572808: the aircraft was late to arrive. We were supposed to depart at XG00. We took over the aircraft and started our preflight preparation. The captain noticed the release did not have the MEL fuel of 1000 pounds that was required. The fueler put on the extra 1000 pounds and we pushed from the gate. After getting airborne we realized we had forgotten to have the flight release amended. The captain called dispatch after our next landing and discussed the release with them.

Title: B737-700 DISPATCHER AND FLT CREW DID NOT ACCOUNT FOR EXTRA FUEL REQUIRED TO COMPLY WITH AN FAA AIRWORTHINESS DIRECTIVE NOTE.

Narrative: DISPATCH RELEASE ERROR. I GOT A CALL FROM ZZZ OPS AGENT ADVISING THAT THERE WAS A DELAY ON FLT TO ZZZ1 DUE TO FUELING. THIS FIGURE IMMEDIATELY RANG A BELL WITH ME, AND WHEN I CHKED THE DISPATCH RELEASE THAT I'D SENT EARLIER, SURE ENOUGH, I HAD FAILED TO INCLUDE THE 1000 LB FUEL REQUIRED BY THE AIRWORTHINESS DIRECTIVE ON THE -700, PLUS A CORRESPONDING REMARK. I RE-RAN THE DISPATCH RELEASE TO INCLUDE THESE ITEMS, AND REVISION-1 WAS SENT AT XH05. I LATER GOT A CALL FROM THE CAPT UPON ARR AT ZZZ1 REGARDING THE DELAY AT ZZZ AND MY FAILING TO HAVE PROPERLY INCLUDED THE AIRWORTHINESS DIRECTIVE FUEL EARLIER, AND I APOLOGIZED FOR MY ERROR. IT BECAME EVIDENT TO ME FROM LOOKING BACK AT HIS TIMES THAT HE HAD NOT RECEIVED THE REVISION-1 THAT I'D SENT EARLIER. MY ERROR NOTWITHSTANDING, I SUGGESTED TO THE CAPT THAT A PHONE CALL COULD HAVE BEEN INITIATED BEFORE DEP FROM ZZZ (WHEN HE FIRST REALIZED THE AIRWORTHINESS DIRECTIVE FUEL HAD NOT BEEN INCLUDED UPON HIS ORIGINAL DISPATCH RELEASE) AND THAT WE COULD HAVE ACCOMPLISHED RELEASE CHANGES VERBALLY. HE MENTIONED THAT HE'D BE FILING A RPT, AND I APPRECIATE BEING INVITED IN AS WELL. CONTRIBUTING TO THE SIT WAS THE FACT THAT MDW HAD BEEN BELOW LNDG MINIMUMS BTWN APPROX XA00 AND XG00 AND THAT NUMEROUS IRREGULAR OPS WERE IN PROGRESS. WHEN I'D TAKEN THE SHIFT AT XC00, I INHERITED 1 MDW DIVERSION (IN PROGRESS) EVENTUALLY ALONG WITH 4-5 OTHER MDW FLTS THAT WERE BEING HELD AT THEIR DEP POINTS. 3 OF THESE WERE ZZZ-MDW FLTS, AND THE OVERALL NUMBER OF PHONE/RADIO CALLS HAD BEEN PRETTY HVY, ESPECIALLY WHEN MDW CAME ABOVE MINIMUMS AND I WAS TRYING TO GET EVERYONE ELSE GOING AT ROUGHLY THE SAME TIME AS THIS FLT'S DEP. SUPPLEMENTAL INFO FROM ACN 572808: THE ACFT WAS LATE TO ARRIVE. WE WERE SUPPOSED TO DEPART AT XG00. WE TOOK OVER THE ACFT AND STARTED OUR PREFLT PREPARATION. THE CAPT NOTICED THE RELEASE DID NOT HAVE THE MEL FUEL OF 1000 LBS THAT WAS REQUIRED. THE FUELER PUT ON THE EXTRA 1000 LBS AND WE PUSHED FROM THE GATE. AFTER GETTING AIRBORNE WE REALIZED WE HAD FORGOTTEN TO HAVE THE FLT RELEASE AMENDED. THE CAPT CALLED DISPATCH AFTER OUR NEXT LNDG AND DISCUSSED THE RELEASE WITH THEM.
