Narrative:

Air carrier Y checked on over jhawk at 12000 ft, gave aircraft instructions for arrival. Within seconds gave traffic alert at 10 O'clock position, then another at 12 O'clock position 1 mi 11500 ft climbing. Then traffic alert with turn, aircraft responded with immediate climb. Aircraft (air carrier X) had reported in clouds. Air carrier Y aircraft was missed by the third aircraft by less than 3 mi and 500 ft. The third aircraft (unknown) was on a XXXX squawk, doing about 360 KTS. Once aircraft missed air carrier X, it descended to 9500 ft and changed to VFR squawk. I tagged aircraft as CBA1, tagged was dropped south of ojc, I retagged aircraft. Third aircraft appeared to land at szl, supervisor said he spoke with pilot and he was VFR and saw air carrier Y.
