Narrative:

On aircraft X apr/xa/02, flight abcd to ZZZ from ZZZ1, #1 thrust reverser failed to deploy because thrust reverser lockout pin was not removed after routine maintenance. Lockout pin has a long (6 ft) 'remove before flight' flag on it. However, flag blows up out of sight when engine is motored.

Title: AN AIRBUS 320 WAS DISPATCHED IN NON COMPLIANCE WITH #1 ENG THRUST REVERSER PINNED IN THE LOCK POS.

Narrative: ON ACFT X APR/XA/02, FLT ABCD TO ZZZ FROM ZZZ1, #1 THRUST REVERSER FAILED TO DEPLOY BECAUSE THRUST REVERSER LOCKOUT PIN WAS NOT REMOVED AFTER ROUTINE MAINT. LOCKOUT PIN HAS A LONG (6 FT) 'REMOVE BEFORE FLT' FLAG ON IT. HOWEVER, FLAG BLOWS UP OUT OF SIGHT WHEN ENG IS MOTORED.
