Narrative:

After a long day, we were in sea the second time. The new aircraft had various maintenance problems which resulted in an additional 1 hour delay (with FAA representative in jump seat!). The takeoff out of sea to anc was ok till at 11000 ft, with the first officer flying, he had a symbol generator failure (lost all attitude reference). Aircraft xferred to me while we followed the checklist and symbol generator failure procedures. We continued to anc, hand flying the 4 hour leg by using the captain's symbol generator displayed to both sides. (Our procedures dictate that with 1 symbol generator out, no autoplt.) also, new checklists in the cockpit, the old landing checklist had been removed from the yoke. So I blew it by not reading the landing checklist from the new checklist kept on top of the glare shield and did it from memory! No excuse.
