Narrative:

Departing on flight abcd we reviewed the maintenance logbook at the gate that had a minor cabin write-up that indicated the captain's PA was 'scratchy.' after departing we realized we did not obtain a balancing entry by maintenance personnel. The previous crew that made the entry did not enter a code in the book, so it may not have been called in to maintenance. In spite of our haste, I did not consider this item to be a serous safety issue, which I believe contributed to my forgetfulness.

Title: B757 FLT CREW OPERATED THE ACFT WITH AN OPEN LOGBOOK MAINT ITEM.

Narrative: DEPARTING ON FLT ABCD WE REVIEWED THE MAINT LOGBOOK AT THE GATE THAT HAD A MINOR CABIN WRITE-UP THAT INDICATED THE CAPT'S PA WAS 'SCRATCHY.' AFTER DEPARTING WE REALIZED WE DID NOT OBTAIN A BALANCING ENTRY BY MAINT PERSONNEL. THE PREVIOUS CREW THAT MADE THE ENTRY DID NOT ENTER A CODE IN THE BOOK, SO IT MAY NOT HAVE BEEN CALLED IN TO MAINT. IN SPITE OF OUR HASTE, I DID NOT CONSIDER THIS ITEM TO BE A SEROUS SAFETY ISSUE, WHICH I BELIEVE CONTRIBUTED TO MY FORGETFULNESS.
