ATC facility: brasilia center. Location: brs 186 degree radial at 20-40 DME. En route, passenger experienced severe chest pain. Doctor said he was having a heart attack. Oxygen and electronic defibrillator were used on him as well as nitro pill. Doctor and flight attendant said he required immediate attention. First officer concurred that he needed immediate attention. We called brasilia center to declare a medical emergency for an immediate landing at sbbr airport and to have medical personnel standing by. We started dumping fuel -- 24000 pounds to get us at a landing weight 336000 pounds. Landing normal except at about 100 KTS. Used hard braking action because the runway had a crest to it, so it looked like we were at the end of runway, when in fact we had about 3000-4000 ft of runway left. At gate, did not set parking brakes, made sure aircraft chocked. I had the relief pilot look at the brakes for hot or glowing brakes and smoke -- none noticed. At about 5-10 mins alter, 1 of 5 fuse plugs went in the next 20 mins. Medical personnel entered aircraft, stabilized the passenger, and removed him. In-flight, called flight support XXX to relay to dispatch our situation. Unable to call dispatch on YYY communication. On ground, could not get flight support XXX on HF.
