Narrative:

Clearance should be forwarded to the pilot at frequency change. From captain's call #1 to the tower, takeoff clrncs, altitudes and heading information should be given. Example: captain: 'maintain 2000 ft, departure 135.4, squawk XXXX.' ground: 'XXXX.' tower: 'fly runway heading, maintain 2000 ft.' departure: 'radar contact, maintain 2000 ft or altitude pilot's discretion,' so everyone will be on the same page and altitude.
