Narrative:

Failed to fly SID. After takeoff from runway 27, failed to turn to 235 degree heading. I and the copilot thought we were not cleared for the SID, so we flew runway heading. During preflight, we discussed the departure and even reread the pre departure clearance between the two of us. Our mistake was not reading the entire printout. He and I read just the flight plan route that was at the top. We had been flying mostly in texas (iah-dfw) and were used to the SID in the flight plan route of the pre departure clearance. Out of bos, the SID was listed on the next to last line. We realize we should read the entire pre departure clearance, but it might be nice to have standard format for all pre departure clearance's. There were no conflicts, just embarrassing for the controller to ask us where we were going.

Title: F100 CREW HAD TRACK DEV IN A90 CLASS B AIRSPACE.

Narrative: FAILED TO FLY SID. AFTER TKOF FROM RWY 27, FAILED TO TURN TO 235 DEG HDG. I AND THE COPLT THOUGHT WE WERE NOT CLRED FOR THE SID, SO WE FLEW RWY HDG. DURING PREFLT, WE DISCUSSED THE DEP AND EVEN REREAD THE PDC BTWN THE TWO OF US. OUR MISTAKE WAS NOT READING THE ENTIRE PRINTOUT. HE AND I READ JUST THE FLT PLAN RTE THAT WAS AT THE TOP. WE HAD BEEN FLYING MOSTLY IN TEXAS (IAH-DFW) AND WERE USED TO THE SID IN THE FLT PLAN RTE OF THE PDC. OUT OF BOS, THE SID WAS LISTED ON THE NEXT TO LAST LINE. WE REALIZE WE SHOULD READ THE ENTIRE PDC, BUT IT MIGHT BE NICE TO HAVE STANDARD FORMAT FOR ALL PDC'S. THERE WERE NO CONFLICTS, JUST EMBARRASSING FOR THE CTLR TO ASK US WHERE WE WERE GOING.
