Narrative:

On final approach, outside OM, cleared for ILS runway 12L, tower advised we were lined up with runway 12R. I had inadvertently dialed in the runway 12R ILS frequency by mistake. We reset radios and continued uneventfully for runway 12L. Since the WX required an approach briefing, I conducted a thorough briefing with the copilot, for the correct runway including the proper frequencys. However, after the briefing, I had to use VOR frequencys to fly the STAR (arrival). My approach plate was in the binder on my left side, and apparently flipped over to the wrong runway (runway 12R). On base leg, when I tuned the ILS, I neglected to confirm the proper runway, just seeing runway 12. I dialed in runway 12R, and idented it with the morse code identify, never realizing it was the wrong runway. Thankfully, approach control handed us over to tower far enough out to correct the problem outside the marker. From now on, when I brief an approach, I will remove the plate from the binder and place it on the clip/chart holder on the yoke.

Title: DC9 CREW LINED UP ON WRONG PARALLEL RWY, WRONG ILS TUNED IN.

Narrative: ON FINAL APCH, OUTSIDE OM, CLRED FOR ILS RWY 12L, TWR ADVISED WE WERE LINED UP WITH RWY 12R. I HAD INADVERTENTLY DIALED IN THE RWY 12R ILS FREQ BY MISTAKE. WE RESET RADIOS AND CONTINUED UNEVENTFULLY FOR RWY 12L. SINCE THE WX REQUIRED AN APCH BRIEFING, I CONDUCTED A THOROUGH BRIEFING WITH THE COPLT, FOR THE CORRECT RWY INCLUDING THE PROPER FREQS. HOWEVER, AFTER THE BRIEFING, I HAD TO USE VOR FREQS TO FLY THE STAR (ARR). MY APCH PLATE WAS IN THE BINDER ON MY L SIDE, AND APPARENTLY FLIPPED OVER TO THE WRONG RWY (RWY 12R). ON BASE LEG, WHEN I TUNED THE ILS, I NEGLECTED TO CONFIRM THE PROPER RWY, JUST SEEING RWY 12. I DIALED IN RWY 12R, AND IDENTED IT WITH THE MORSE CODE IDENT, NEVER REALIZING IT WAS THE WRONG RWY. THANKFULLY, APCH CTL HANDED US OVER TO TWR FAR ENOUGH OUT TO CORRECT THE PROB OUTSIDE THE MARKER. FROM NOW ON, WHEN I BRIEF AN APCH, I WILL REMOVE THE PLATE FROM THE BINDER AND PLACE IT ON THE CLIP/CHART HOLDER ON THE YOKE.
