Narrative:

Crew was aboard the airplane preparing for departure when the gate agent came into the airplane with our copies of the hazardous cargo manifest. Dry ice was being placed in the airplane -- the location of which was not designated on the form. Furthermore, the forms did not have the required 3 signatures. The forms got signed and it was determined that there were no live animals in the forward cargo compartment where the dry ice was to be placed. We subsequently departed for dfw. After takeoff, we noticed that the close-out did not reflect there were restr items in the forward cargo compartment. Shortly thereafter, we had to divert into msy. As we were coming into msy, an updated close-out came over the ACARS. Perhaps the ground personnel could take a moment to review the procedures required when hazmats are to be loaded onto the airplane. We, the crew, should have noticed that they were not on the closeout before takeoff. We did, however, know that the items were aboard, in the forward cargo compartment and that there were no live animals in that compartment.

Title: AN ACR DEPARTS WITHOUT PROPERLY COMPLETED RESTR ARTICLES PAPERWORK.

Narrative: CREW WAS ABOARD THE AIRPLANE PREPARING FOR DEP WHEN THE GATE AGENT CAME INTO THE AIRPLANE WITH OUR COPIES OF THE HAZARDOUS CARGO MANIFEST. THE FORMS GOT SIGNED AND IT WAS DETERMINED THAT THERE WERE NO LIVE ANIMALS IN THE FORWARD CARGO COMPARTMENT WHERE THE DRY ICE WAS TO BE PLACED. WE SUBSEQUENTLY DEPARTED FOR DFW. AFTER TKOF, WE NOTICED THAT THE CLOSE-OUT DID NOT REFLECT THERE WERE RESTR ITEMS IN THE FORWARD CARGO COMPARTMENT. SHORTLY THEREAFTER, WE HAD TO DIVERT INTO MSY.
