Narrative:
I completed MD80 transition training and was issued a temporary airman certificate on may/xa/00. I sent a written change of address to the FAA airman certification branch in july with my new address and gave 1 copy to the chief pilot's office and placed 1 copy with my temporary certificate. I moved to a new home in virginia oct/xa/00. I pulled out my certificate and medical today while starting my FAA class one medical, and saw that my temporary airman certificate expired september. I have flown 3 scheduled sequences in this condition. I kept both the flight office and the FAA informed of my address. I have received FAA airman's information circulars forwarded to my new address. I have no idea why I have not received the permanent airman certificate. I was obviously distracted somewhat with 2 house closings, 2 moves, 1 cross country, and learning a new base and aircraft. I expected the system to work and the check I had to remind me, a note in my computer for a date in july, was somehow erased.

Narrative: I COMPLETED MD80 TRANSITION TRAINING AND WAS ISSUED A TEMPORARY AIRMAN CERTIFICATE ON MAY/XA/00. I SENT A WRITTEN CHANGE OF ADDRESS TO THE FAA AIRMAN CERTIFICATION BRANCH IN JULY WITH MY NEW ADDRESS AND GAVE 1 COPY TO THE CHIEF PLT'S OFFICE AND PLACED 1 COPY WITH MY TEMPORARY CERTIFICATE. I MOVED TO A NEW HOME IN VIRGINIA OCT/XA/00. I PULLED OUT MY CERTIFICATE AND MEDICAL TODAY WHILE STARTING MY FAA CLASS ONE MEDICAL, AND SAW THAT MY TEMPORARY AIRMAN CERTIFICATE EXPIRED SEPTEMBER. I HAVE FLOWN 3 SCHEDULED SEQUENCES IN THIS CONDITION. I KEPT BOTH THE FLT OFFICE AND THE FAA INFORMED OF MY ADDRESS. I HAVE RECEIVED FAA AIRMAN'S INFO CIRCULARS FORWARDED TO MY NEW ADDRESS.
